Agency Creek Road begins as a paved route in the Grande Ronde Agency, branching off Highway 22. After about 7.5 miles, it reaches a Y intersection, veering north and transitioning into a gravel surface. The road then winds through the Siuslaw National Forest, eventually connecting with the Mt. Hebo road. It traces the path of Agency Creek, climbing toward the ridge's summit. Most of the route is gravel, with the occasional pothole dotting the way. On the northern end, the road changes names to Agency Ridge Road.
